1t reads to me as 1962 Jazz bass Road Worn Volume (not sure what the W is) Tone With/Case.

That sounds right, except I thought they'd changed to VVT from the stacked controls by then?

I think in the fender world they just call stacked jazzes '62's, as they tend not to every year model
